Per the terms of my lease, I agree that any repair performed resulting from Tenant negligence, abuse or neglect shall be paid by the Tenant. I authorize the release of my contact information and premise key for the purpose of allowing an authorized maintenance person to service this request during the hours of 8am through 7pm. If an appointment is required, I agree that the service provided will be entitled to a $50.00 trip charge, at my expense, if this appointment is not kept or rescheduled a minimum of 24hours prior to the appointment.
